JOINT NOTICE OF PRIVACY PRACTICES REVISION
This notice describes how protected health information about you may be used and disclosed and how you can get access to this information.
Effective February 4, 2013, South County Hospital Healthcare System has revised our Notice of Privacy Practices to include employed physician practice groups who may share protected health information with each other through an electronic record system separate and apart from the hospital’s electronic system. These computer systems each allow healthcare providers to electronically share and access your protected health information when you are treated at the hospital or at one or more of the South County Hospital Healthcare Systems employed physician practice groups. You may request a copy of the revised Notice of Privacy Practice at the time of your visit, obtain a copy from our website at www.schospital.com or call the SCHHS Patient Access Department and request that a revised copy besent to you in the mail.
